2. Copies of all commercial public liability insurance policy certificates held by the Ministry of Forests, including what is covered, policy numbers, providers and underwriters. Please include all documents in these policies that mention: BTK, Bacillus thuringiensis, Bt, gypsy moth, spongy moth, Lymantria, Lepidoptera, caterpillar, larvae, Geometridae, spraying, aerial, pesticide.
3. A complete list with percentages of ALL the ingredients besides the 2% Btk in the proposed spray.
